Combining experimental surgery and translational research for better individual treatment
Malignant melanoma is a rapidly increasing cancer, and Sweden has the 4th highest incidence in Europe, with approximately 4000 new patients yearly. Different types of melanoma are caused by different genetic mutations with different incidence, localization and clinical characteristics. Cutaneous melanoma is the most frequent encountered form. The incidence is similar worldwide between men and women, even if the localization is different. Women more often present with melanomas on the lower extremities, while it is more common with trunk melanomas in men.

- The overall aim of the thesis was to analyse the outcomes of four studies representing the ongoing evolution in clinical and experimental surgery to improve outcome for patients with melanoma, says Valerio Belgrano, PhD-student at the Department of Surgery, at Sahlgrenska Academy.

Sentinel node status, tumour thickness and ulcerated - important prognostic factors
The first paper studied the outcomes of all sentinel node* biopsies (SNB) performed at Sahlgrenska University Hospital, and this verified that the most important prognostic factors for survival, were sentinel node status, the tumour thickness and if the melanoma tumour was ulcerated.

Further the results of isolated limb perfusion (ILP) for melanoma in-transit metastasis was studied. The technique is a procedure where a limb is surgically isolated from the rest of the body, connected to a heart-lung machine, and then very high doses of a chemotherapeutic agent is perfused through the limb for one hour.

- The first study showed that in the case of recurrence it was safe to repeat the procedure. We could also show that if the treatment worked the first time, it was very likely that the patient would respond after a repeated procedure. Another question concerned the importance of BRAF-mutation as a predictive factor for response, since there were earlier conflicting results. We were able to show that BRAF-status do not affect response of the treatment.

- Finally, we created patient-derived xenograft models from 21 cutaneous melanoma biopsies transplanted into advanced mouse models and we could show that the models could be used to predict the effect immunotherapy in patients.

The conclusions are; that the surgical approach and therapies for patients with cutaneous melanoma are becoming more targeted and personalized. A specialised multidisciplinary approach can improve the understanding of the disease, support the decision-making process towards the most advantageous treatment options for each individual patient at a specific time.

*Sentinel node is, by definition, the first lymph node that drains the skin area where the primary tumor sits and is therefore the best place to selectively look for metastasis
